Wound Care Treatment and Follow upImage result for wound management

GPPC has successfully tendered to provide assessment services for "wound care, treatment and follow up" in NE Essex. This service has been provided in general practices in the past either through individual contracts with the local Clinical Commissioning Group (CCG) or by direct referral to hospital. From 1/7/16, GPPC has been contracted by the CCG to provide a community based service which GPPC will do through it's member practices. This is part of the general move for patient services to be "closer to home", providing quicker and easier access in a safe, well organised system.

Although General Practice has always provided wound care management to patients presenting to the practices after procedures performed in General Practice, the follow up to hospital operations has normally been done in secondary care. The CCG has now commissioned General Practices to take on this service and the majority of local practices have agreed to take part. For some practices, patients may still be referred back to hospital for their wound care.

This service includes:

  • Suture, staple and clip removal from post-operative and traumatic wounds.

  • Monitoring of wounds (post-operative and traumatic) as required clinically and requested by secondary care teams

  • Re-dressing or packing of post-operative and traumatic wounds initiated in hospital.
